Ozempic vs Wegovy vs Mounjaro: What's the Actual Difference?

If you've spent any time researching GLP-1 medications, you've probably noticed the names get used almost interchangeably online — as if Ozempic, Wegovy, and Mounjaro are just different flavours of the same thing. They're related, but the differences between them actually matter, especially when it comes to what they're licensed for, how they're dosed, and what the clinical trial data says about results.

Ozempic and Wegovy: same drug, different dose, different licence

Ozempic and Wegovy both contain semaglutide, made by Novo Nordisk (Ro, 2024). The difference isn't the drug — it's the dose and what it's licensed for.

Ozempic is licensed for type 2 diabetes, typically dosed at 0.5–1 mg weekly, and produces weight loss as a secondary effect rather than its primary purpose (Doctronic, 2025). Wegovy is the same molecule at a higher maintenance dose — 2.4 mg weekly — and is specifically licensed for chronic weight management (Doctronic, 2025; Ro, 2024). In its pivotal trial, Wegovy produced an average 15% body weight reduction over 68 weeks (Doctronic, 2025).

That difference in licensing is exactly why insurance and NHS access criteria differ between the two, even though the active ingredient is identical (Doctronic, 2025).

Mounjaro: a different molecule entirely

Mounjaro contains tirzepatide, made by Eli Lilly, which is a genuinely different mechanism — it activates both the GIP and GLP-1 receptors, rather than GLP-1 alone (Ro, 2024). Mounjaro is licensed for type 2 diabetes, with approved doses ranging from 2.5 mg up to 15 mg weekly (Scale Solutions, 2026).

The dual-receptor mechanism isn't just a marketing point — it shows up directly in head-to-head trial data. The SURPASS-2 trial compared tirzepatide against semaglutide 1 mg in people with type 2 diabetes: tirzepatide at 10–15 mg produced 7.6–11.2 kg of weight loss and HbA1c reductions of 2.01–2.30%, compared with 5.7 kg and 1.86% for semaglutide 1 mg (SURPASS-2, cited in Rathmann et al., 2023). Side effects — nausea, diarrhoea, reduced appetite — occurred at broadly similar rates across both groups (Rathmann et al., 2023).

At the higher, weight-management doses, the gap holds up too. The SURMOUNT-5 trial, a head-to-head comparison in adults with obesity but without diabetes, found tirzepatide produced significantly greater weight loss than semaglutide, along with larger improvements in blood pressure, HbA1c, fasting insulin, and cholesterol markers at 72 weeks (Ard & Fitch, 2025).

Why the differences actually matter to you

Licensing affects access. Wegovy is licensed for weight management, so it's the option prescribers and insurers typically consider for that purpose. Ozempic and Mounjaro are licensed for diabetes — using either off-label for weight loss alone is a decision to make with a prescriber, not a pharmacy website (Scale Solutions, 2026).

Dose isn't a fixed number — it's titrated. All three medications start at a low dose and increase gradually over weeks or months, specifically to reduce the nausea and GI side effects that come with rapid dose escalation (Ro, 2024).

More average weight loss isn't the same as more weight loss for you. Trial averages describe group outcomes, not individual results. Response to any GLP-1 or dual-agonist medication varies considerably from person to person, and side effect tolerance is just as relevant as the headline percentage.

What none of these medications do on their own

Whichever of the three you're on, the mechanism is the same story: appetite suppression and slower digestion, not a nutrition plan or a strength routine built for you. The dose differences and trial percentages above describe what happens to body weight — they say nothing about whether that weight loss is coming from fat or muscle, or what happens once you eventually come off.

The bottom line

Ozempic and Wegovy are the same drug at different doses for different licensed uses; Mounjaro is a different molecule targeting two hormone receptors instead of one. The trial data consistently shows tirzepatide producing larger average weight and blood sugar improvements than semaglutide at comparable points in treatment — but licensing, access, individual response, and side effect tolerance all matter more to your actual outcome than which name is on the pen.
